All-on-4 Dental Implants (upper and lower arch) Before & After Case 1
This patient grew up with perfect teeth and a beautiful smile, which all changed after being diagnosed with an autoimmune disease that gave her severe dry mouth and led to tooth decay. This led the patient to undergo multiple extractions over many years, trying to hold on to her failing front teeth to avoid embarrassment. She visited our office, and we took a CT scan, which indicated that she would be a good candidate for all-on-four immediate implant placement and immediate tooth delivery for both jaws. Financial constraints led the patient to opt for an implant-supported prosthesis in the lower jaw and a complete denture in the upper jaw. At the single surgical appointment, we extracted the hopeless teeth and used a bone graft to preserve her remaining bone, placed 4 implants in her lower arch, and delivered immediate temporary teeth in the same appointment. Giving the patient these temporary immediate prostheses allowed her to walk out of the appointment with a complete set of new teeth. After 4 months of healing, the patient received a final implant-supported zirconia prosthesis for the lower jaw and a new denture for the upper jaw. Now, the patient finally remembers what it’s like to have a beautiful smile, and she can’t stop smiling in pictures with her grandkids.

Many years of severe tooth wear led to a hopeless dentition and collapsed bite in this patient. After being told by multiple dentists that he could not have dentures made due to the size of his mandibular tori, he thought he was out of options and would not be able to chew comfortably for the rest of his life. During his first visit to our office, we took a CT scan and determined that he would be a good candidate for “all-on-four” implant-supported fixed prostheses in his upper and lower arches. With digital scanning and smile design technology, we planned his restorations before his surgery. At his next appointment, we extracted his teeth, placed 4 implants in each arch, and immediately delivered his new prostheses. The patient left our office transformed, with a new complete smile that complimented his facial features. This patient had a history of severe periodontal disease and avoided seeing a dentist for many years. She resorted to gluing her failed bridge back into her mouth each day, just so she could look like she had teeth, but she could not chew anything at all. After coming to our office and having a CT scan done to evaluate her bone levels and determine her eligibility for implant placement, we planned an all-on-four hybrid prosthesis for her lower arch and a complete denture for her upper arch. In one surgical visit we completed her extractions, placed implants, and delivered a complete set of prostheses. The patient was thrilled to leave the office with a brand-new set of teeth in the same day.

The patient saw multiple dentists, looking for an option to replace his teeth, but was only given options for removeable dentures, which he could not tolerate. After looking for a dentist that would give him an option for fixed prostheses that he would not have to take out of his mouth, he came to our practice. During his visit, we implemented our diagnostic workflow and digital planning, ultimately developing a treatment plan that involved the placement of multiple dental implants to restore the patient’s function and improve his quality of life. In a single surgical visit, we extracted his hopeless teeth, placed 4 implants in each arch, and immediately delivered fixed prostheses that he would not have to remove.

This patient presented with a failing dentition caused by years of tooth decay. Over the years, she underwent multiple extractions and tried several times to restore her teeth. The patient was left with a bridge and removeable dentures placed on 2 implants. She was left feeling unhappy with both her appearance and daily function, having to take out her dentures every day. In our office we took a 3-D scan to evaluate her bone levels, and used digital smile design technology to prepare for precise implant placement and to design both temporary and permanent prostheses for the patient. Our use of digital technology allowed us to include the patient in the design process and fabricate her prosthesis prior to her surgery. With a finalized treatment plan in place, we treated the patient in 1 surgical appointment which included extractions of her remaining upper teeth, placement of 2 additional dental implants, and delivery of an immediate fixed temporary prosthesis, all in the same day. After 4 months of healing the patient received a finalized permanent zirconia prosthesis, and finally felt confident in her appearance.
