This patient presented with a crown that had fallen out of her mouth earlier that day during a business meeting. The patient was in distress about having a missing tooth, so our goal was to provide the patient with a solution that day. After examining the patient, we found that her tooth had fractured under the level of the bone, so it would need to be extracted and would require extensive bone grafting to build up the foundation for future implant placement. After the exam, we immediately performed the extraction with simultaneous bone augmentation and provided the patient with a fixed temporary restoration so she would not have any concerns about walking around with a missing tooth. After allowing the grafted site to heal for 5 months, we placed an implant, which was restored with a crown. The patient was extremely grateful for never having to leave our office without a tooth in her mouth.

This patient is a medical resident who was going to lose two posterior teeth due to failing root canal therapy, and was told by another dentist that he should receive a 4-unit conventional bridge to restore the area. When we consider the patient’s young age, the longevity of a conventional bridge (about 15 years), and the need to drill into healthy teeth to support a bridge, we prefer to build the foundation of the site with both hard and soft tissue grafting followed by individual implant placement in both sites. This surgical treatment plan obviates the need to drill into otherwise healthy teeth, allows for better patient-performed hygiene of the area (can floss between each tooth), and has individual implants that allow the biting forces to be distributed more evenly than with a bridge.

This patient had a wide smile and was not happy with the appearance of spaces in the back that always showed in photos. After doing a functional and aesthetic wax-up, we decided to replace the missing teeth with implants and place veneers on the front teeth, ultimately creating a harmonious and aesthetic smile for the patient. After the treatment was complete, he finally felt confident to smile in pictures again. In cases where patients are missing back teeth, it is critical to rebuild function and support of posterior teeth prior to addressing any anterior aesthetic concerns to ensure the longevity of all the prostheses.

This patient presented with a failing crown due to incomplete root canal therapy done a few years prior. In a single surgical visit, we extracted his tooth, grafted the site with bone, and placed an implant. After 3 months of healing, we delivered a final crown to the patient, restoring his oral health, function, and comfort.
