This patient presented with a failing implant bridge and prosthesis and severe periodontal disease on the remaining teeth. Comprehensive diagnostic imaging (CT scan) revealed significant bone loss in both the upper and lower jaws. However, the patient would only be satisfied with something fixed to replace the missing teeth. Given the complexity of the case, we elected to stage his treatment into different phases. We addressed his lower jaw first by removing the failing teeth and implants. We simultaneously placed new implants and bone grafts with immediate temporization, all in the same visit. The patient required bilateral sinus lifts in the upper jaw to place future implants in those areas. After 6 months of healing, 6 implants were placed in the upper jaw. After healing and osseointegration of the implants, a new fixed prosthesis was placed on the upper implants, and the remaining front teeth were extracted. The result was an all-on-six fixed implant restoration for the upper arch and an all-on-4 fixed restoration on the lower arch. Due to the significant history of periodontal disease, we placed the patient on a 4-month maintenance schedule to ensure his continued periodontal health, and the patient has been entirely compliant.

This young patient presented with a severely compromised smile with misplaced and missing teeth in both jaws. Diagnostic imaging revealed severe bone loss, which required a bone augmentation procedure at the time of implant placement. To restore both function and aesthetics, we planned single implants at the canine sites, supported by 2-unit cantilevered bridges on both sides and porcelain crowns for the 2 front teeth. The patient was seeing an orthodontist to properly align the misplaced teeth in the arch. We utilized her orthodontic treatment as a means of temporization throughout treatment. Following successful orthodontic treatment, final restorations were inserted, resulting in a natural, harmonious smile.

This patient presented with severe dental anxiety and multiple implant fractures along with peri-implantitis. Previous dental trauma left the patient hesitant to address his infections, and he did not seek treatment for 3 years. Finally, he came to our office, where we did a comprehensive evaluation with CT imaging and a digital impression and planned an all-on-6 implant-supported fixed prosthesis. It was vital for us to address the patient’s infections, but we also knew the patient would not be satisfied if he left our office without teeth. As such, in a single surgical visit, we extracted his failing teeth and implants, removed the infections, and performed simultaneous bone grafting with implant placement. In that same visit, we inserted a temporary fixed restoration that we made prior to the surgery using the digital scan we obtained at his initial visit. The patient could not believe that he had walked out of that one surgery, looking better than he had when he had walked in. After a six-month healing period while the patient wore the temporary prosthesis, we delivered the final implant-supported zirconia prosthesis.

This patient presented with an old partial denture on the upper arch, which she had been wearing for 35 years. What significantly complicated this case was the patient’s very deep bite, which reduced the available space and limited our treatment options. Fortunately, our team is quite familiar with complex cases such as this one. We performed a comprehensive diagnostic work-up, necessary to determine how much we could open her bite without compromising her comfort and TMJ health. We then performed bilateral sinus grafting procedures in the upper jaw to augment the bone volume needed for implant placement in those areas. After 6 months of healing, 6 implants (3 per side) were placed to support 2 fixed implant bridges (1 per side). After completing her surgical treatment, we finalized her aesthetic transformation by placing porcelain veneers and crowns on the front teeth. In the end, she had a new, harmonious smile that was properly supported by her back teeth, ensuring longevity for the new implants and restorations.

This patient presented a chief complaint that her removable maxillary implant-supported denture would not stay in her mouth, and she was very frustrated. A CT scan revealed that the existing implants were mini-implants, 5 out of 8, which had fractured within her bone, indicating of an improper previous treatment plan, given the patient’s adequate bone volume for conventional implant placement with the use of modern surgical techniques. After developing an appropriate treatment plan, we removed the fractured mini-implants and placed 6 standard dental implants with immediate load with a fixed provisional prosthesis, all completed during 1 surgical appointment. After 4 months of healing, the patient received a brand-new zirconia all-on-six final prosthesis.

This patient presented with a multitude of dental challenges including broken implant crowns, a deep bite, a recurrent caries under existing crowns. Complicating matters further, the patient was a heavy bruxer. A functional and aesthetic wax-up was made as a blueprint for her future all-on-six dental implant prosthesis in the upper arch. We utilized her existing implants in the lower arch along with new crowns and veneers to open her bite, restoring a proper occlusion for a stabilized bite while also giving the patient a beautiful new smile.
