Immediately following the accident, she was seen by a dentist, who removed the remaining roots of 2 of her front teeth, placed a bone graft, and instructed the patient to wear a removeable partial denture for several months while the bone graft healed. Dissatisfied with this very unaesthetic solution, the patient visited our office within 1 week of the accident, looking for a 2nd opinion. After a thorough evaluation and diagnostic work-up of the patient, we planned a guided implant surgery with the placement of a temporary tooth that is fixed in place, along with a soft tissue graft to optimize the esthetic results of the surgery. Most important to the patient was that this was all done in the same appointment, and she would not have to take a removable partial out while eating or sleeping, and she could smile and function normally immediately. 4 months later, we delivered the finalized restorations, 2 implant crowns, and 2 veneers.

This patient came to our office complaining of discomfort in her front tooth and she noted that she was not happy with the color of her all-on-4 teeth, which had been veneered by another dentist a few years prior. The patient was also able to complete Invisalign treatment to straighten her teeth, all while the implant healed and she functioned normally with the temporary implant crown. 8 months later, when her orthodontic treatment was complete, we delivered a final implant crown with no downtime for the patient.

This patient came to our office complaining of slight discomfort in her canine tooth when it touched anything solid. A CT scan revealed a large infection along with root resorption of the canine tooth, which would need to be extracted. Due to the extent of bone resorption from the infection, immediate implant was not an option. At the time of extraction, we performed bone and soft tissue grafting to preserve the architecture of the site for eventual implant placement and inserted a temporary fixed restoration (maryland bridge), so the patient would have a tooth during the healing process. After 4 months of healing, we had a healthy site with enough bone and healthy soft tissue to place an single tooth implant using a surgical guide along with a temporary tooth fixed to the implant. In some instances, a staged approach is necessary to give the patient a predictable and aesthetic result, which we achieved in this case.

This patient was undergoing an extensive treatment plan with her general dentist to place crowns and veneers on all her teeth when her lateral incisor broke beyond repair and her general dentist did not feel comfortable treating it herself. A CT scan of her dentition revealed that more than half of the root of her front tooth had undergone resorption, essentially eaten away under the bone. Adding to the complexity of the case was her high smile line, which creates one of the most difficult aesthetic challenges when treating the front teeth. Thankfully, however, this is our specialty, and we know how crucial it is to place a temporary tooth immediately to properly mold the patient’s gums and match the rest of her teeth. We extracted the front tooth and placed an immediate implant along with bone grafting and soft tissue grafting using the patient’s own tissue. We then placed a temporary restoration immediately onto the implant made from the patient’s own extracted tooth! We let the implant heal for 5 months before placing a permanent implant crown, which had perfect soft tissue and bony contours because of our meticulous treatment planning.

This patient had a case of external root resorption on her front tooth, which made it necessary to extract this tooth. Through our treatment planning process we obtained a CT scan, which allowed us to plan for an immediate implant to replace the missing tooth. Due to the degree of resorption and the resultant bony defect, the implant needed time to heal under the gums before being loaded with a permanent tooth. Even though we could not place a screw-retained temporary tooth on the implant, it is vital to mold the soft tissue during the healing period, so we made a custom healing abutment for the implant that would do just that. We also inserted a temporary Maryland bridge that would replace the missing tooth in the meantime, while the implant integrated. After 4 months, a definitive screw-retained implant crown was delivered, with an aesthetic, functional, and healthy result.
