This patient presented to our office with advanced periodontal disease after being told by a previous dentist that she was not a candidate for implants. She was very self-conscious about her smile, stating that her family has started mentioning it to her. After a comprehensive work-up with 3D imaging, the patient was treatment planned for 4 implants to be placed in both arches. On the date of surgery, all tooth extractions were done along with extensive bone grafting, and temporary restorations were given to the patient on the same day. The patient healed for 5 months, after which we delivered her a new final prosthesis. Now, she smiles with her grandkids and couldn’t be happier.

This patient presented with severe dental phobia, which had led her to avoid any dental care in the last 10 years. She finally worked up the courage to make an appointment with our office, desperate to fix her smile so she could get a job and enter the workforce. Complicating the case was this patient’s severe gag reflex, which made even taking X-rays very difficult. Under IV sedation, all the teeth were extracted and we placed 5 implants in the lower jaw, while performing bilateral sinus lifts in the upper jaw. After 6 months of healing, 6 implants were placed in the upper jaw, and the patient was given a fixed prosthesis, fully supported by implants. We went through 4 different sets of temporaries so we could perfect her smile and everyday function, as well as ensure patient satisfaction.

This patient presented with an extremely deep bite, advanced periodontal disease, and multiple teeth had already fractured. A comprehensive treatment plan was developed to restore his function and aesthetics. This treatment plan consisted of an all-on-6 implant-supported restoration in the maxilla and an all-on-5 implant-supported restoration in the mandible. The patient received fixed prostheses on the same day of surgery. After 5 months of healing and 3 sets of new temporaries, the patient received his final prostheses, which fit perfectly.

This patient had not seen a dentist in over 20 years. She presented to our office with extreme anxiety, hesitant to even open her mouth due to fear of judgment over the status of her dentition. Through some coaching and encouragement, we were able to complete our examination, which demonstrated complete neglect and a total lack of oral hygiene for many years. We formulated a treatment plan that focused first on improving her oral hygiene and lowering the bacterial content in her mouth to ensure a more optimal surgical environment for our future treatment. The surgical treatment plan involved placing 6 implants in the upper jaw and All-on-4 dental implants in the lower, with immediate temporaries given at the time of surgery. Throughout the normal healing process the soft and hard tissue change, and we constantly re-evaluate the patient’s bite, comfort, and aesthetics with the temporaries so that all adjustments can be made prior to fabricating a final prosthesis that fits perfectly. We do this as many times as necessary, usually dependent on individual patient healing patterns, ultimately providing the patient with proper function, aesthetics, and patient satisfaction.

This patient works as a realtor and constantly must interact with clients in a public setting. Her primary concern was that her teeth were beginning to affect her business, as she had become too embarrassed to show her teeth. Additionally, she mentioned her difficulty with chewing due to the looseness of her teeth. Ultimately, her goal was to have a perfect smile with straight teeth and a healthy mouth. After a comprehensive exam and CT scan, we developed a treatment plan that included the placement of 6 dental implants in the maxilla and 4 implants in the mandible, with simultaneous bone grafting to rebuild the bone that had been lost due to years of untreated periodontal disease. At the time of surgery, the dental implants were placed and immediate temporaries were delivered to the patient. A series of new temporaries were fabricated over several months to accommodate for the changes that occur throughout the healing process. This consistent re-evaluation lets us ensure that the patient is functioning properly with her new bite and new teeth, and it also lets the patient take part in the process of finalizing her new teeth, ensuring the patient’s satisfaction with the fit, function, and aesthetics of the final restorations.

This patient came to our office with an upper bridge, which he said kept falling out of his mouth. A comprehensive exam revealed that he had severe periodontal disease and none of his teeth could be saved. We created a treatment plan that would stage treatment of the upper and lower arches, so he would always have teeth in his mouth. First, we extracted his upper teeth and placed 6 implants in the upper jaw along with simultaneous bone grafting to compensate for the bone loss that had been worsening for many years due to his failing teeth. In that same visit, we gave the patient a temporary prosthesis, so the patient would have teeth in his mouth throughout the entire healing process. In a separate visit, we followed a similar process for his lower jaw, and the patient walked out of the appointment with new temporary teeth that same day. We allowed the patient to heal for 6 months before making him new, finalized prostheses for both the upper and lower jaws. Utilizing these temporary restorations allows us to analyze how well the patient is functioning daily, and we often make periodic adjustments and new sets of temporaries, which aids in the precise design of the final zirconia restorations. Such a complicated and extensive case often requires this type of phased treatment plan over several months to ensure longevity of the implants and restoration along with patient comfort and satisfaction.
